This website emails the information you type into the contact form to referral@nhhhtx.com so our intake team can return your call. Do not submit Social Security numbers, full medical records, or other sensitive identifiers through the web form.

To request a Notice of Privacy Practices, or to exercise rights regarding your medical record, call the office or write to the administrator at our Mesquite address.
